While the effect of TRT on sleep apnoea is not well understood, one article in The World Journal of Men’s Health says that large doses of testosterone can trigger or exacerbate symptoms by causing physiological changes to the respiratory system. In particular, TRT impacts the mechanisms that regulate oxygen and carbon dioxide levels during sleep. As TRT Australia’s guide explains, exogenous testosterone can make the body less responsive to hypoxia (decreases in oxygen) and hypercapnia (elevated carbon dioxide), potentially creating longer pauses between breaths. Larger doses of testosterone may also heighten the body’s metabolic rate, inducing a higher demand for oxygen and compounding the effects of any disruptions to breathing at night.
Additionally, TRT Australia’s guide says that testosterone can alter fat distribution around the throat and decrease pharyngeal muscle tone, potentially leading to the narrowing or collapse of airways. This, in turn, can create obstructions, resulting in obstructive sleep apnoea.
Despite research from various independent studies suggesting a link between TRT and sleep apnoea, one study in the Journal of Clinical Endocrinology and Metabolism found that TRT improved breathing capacity, while a randomised controlled trial in Androgens: Clinical Research and Therapeutics found that long-term TRT at appropriate doses reduced nocturnal hypoxia. While these findings suggest greater complexity in the relationship between testosterone and breathing, TRT Australia says that certain factors, including obesity, age, and pre-existing conditions like chronic nasal congestion or enlarged tonsils, can nevertheless increase an individual’s risk of sleep apnoea.
TRT Australia adds that the combination of TRT and sleep apnoea can cause polycythaemia (blood thickening) and cardiovascular disease. As a result, its guide says that individuals considering TRT must speak to a medical professional to determine whether the treatment is appropriate for them.
